What Is the Average Girth? What the Clinical Data Shows

Average erect girth is 4.59 inches (11.66 cm). That figure comes from a 2015 systematic review in BJU International by Veale and colleagues. The review pooled up to 15,521 men whose measurements were taken by health professionals rather than reported by the men themselves — but only 381 of them were measured erect at the circumference, so this is the thinnest of the review’s numbers. The same review put average flaccid circumference at 3.66 inches (9.31 cm, from 9,407 men), and average erect length at 5.17 inches (13.12 cm, from 692 men).
That is the answer. The rest of this article is about why so many numbers you see online disagree with it — and most of the disagreement comes down to two things: people measuring diameter and calling it girth, and people measuring at different spots on the shaft without saying which.
Where the 4.59-inch figure comes from
Veale’s team went through the published literature and kept only studies where a clinician did the measuring under a defined protocol. That filter is the whole point. Self-reported size data runs consistently higher than clinically measured data, for reasons that need no explanation. When you see a “study” quoting an average erect girth of five inches or more, check whether anyone but the participant held the tape.
The review reports a standard deviation of roughly 1.1 cm (0.43 in) for erect circumference. In plain terms: the bulk of men sit within about half an inch either side of 4.59 inches. The distribution is tight. That is why Veale’s team published nomograms — percentile curves — instead of just a mean. A number on its own tells you nothing; a percentile tells you where you sit in a distribution that turns out to be narrower than most men picture it.

Two honest caveats about the pooled figure: only 381 men underlie it, and the underlying studies did not all measure circumference at the same point on the shaft. Some used mid-shaft, some used the base. That variation is baked into the 4.59-inch average and is a real limitation of it — which brings us to the two problems worth more of your attention than the number itself.
Girth is circumference, not diameter
This error turns up constantly, and it produces wildly wrong conclusions in both directions.
Girth means circumference — the distance all the way around the shaft. You get it with a flexible tape measure or a strip of paper you then lay against a ruler. You cannot get it with a ruler alone.
Diameter is the width straight across, which is exactly what a ruler gives you when you hold it against the shaft.
The two are related by π:
Circumference = diameter × 3.14
Run the average through it. An erect girth of 4.59 inches (11.66 cm) means a diameter of 4.59 ÷ 3.14 = 1.46 inches (3.71 cm).
Now look at what happens when the two get swapped. A man lays a ruler across his erection, reads 1.5 inches, compares it to “average girth 4.59 inches,” and concludes he is dramatically below average. He is not — 1.5 inches of diameter is 1.5 × 3.14 = 4.71 inches of circumference, slightly above the clinical mean. He has just compared a width to a circumference, which is like comparing your waist measurement to how wide you are across the hips.
The error runs the other way too. Someone reads “five-inch girth” in a forum and pictures something five inches across. Five inches of circumference is 1.59 inches across — about the diameter of a shot glass. Once you have the conversion in your head, a lot of the numbers floating around this topic stop sounding alarming.
If your tape reading and your ruler reading are roughly a factor of three apart, that is not an inconsistency. That is π doing its job.
Where you measure changes the answer
The shaft is not a uniform cylinder. In most men the base reads largest, mid-shaft sits somewhere in between, and the point just below the glans is often the narrowest. So a girth figure without a stated measuring point is not comparable to any other girth figure.
Mid-shaft is the working standard. Fully erect, tape wrapped around the middle of the shaft, snug enough to sit flat against the skin but not tight enough to compress anything. If the skin dimples under the tape, you are reading low.
How big is the base-to-mid-shaft gap? There is no good published figure for the typical difference, and anyone who quotes you one precisely is guessing. What matters practically is that the gap exists and that it is large enough to move your percentile. Pick a spot, write down which spot it was, and use the same one every single time. A measurement you can repeat beats a measurement that flatters you.

Flaccid girth tells you less than you think
The review’s flaccid circumference figure — 3.66 inches (9.31 cm) — is real data, but it is close to useless as a personal benchmark. Flaccid size moves with temperature, stress, time of day and how recently you exercised. The flaccid state is also a poor predictor of erect state at the individual level: some men change a great deal between the two, some barely at all, and there is no way to look at one and know the other.
If you are tracking anything over time, track erect measurements. Flaccid numbers mostly generate noise you will then over-interpret.
Girth vs. length: what the evidence actually supports
This is where most articles start making claims they cannot back. Here is what exists.
On the anatomy side: Veale’s team published separate nomograms for length and circumference rather than a single combined size score. Knowing one does not tell you the other with any precision — they are two different distributions, and a man can sit at very different percentiles on each.
On the preference side, the evidence is thin. The most-cited study is Prause and colleagues in PLOS ONE (2015), which had 75 women select among 3D-printed models. Selections averaged modestly above the population mean in both dimensions, with slightly different choices for a hypothetical one-time versus long-term partner. Take that for what it is: 75 participants, one lab, plastic models rather than partners, and a self-selected sample. It is a data point, not a finding you should reorganize your self-image around. Anyone telling you confidently that “women prefer girth over length” is not citing better evidence than this — because better evidence does not exist.
The largest relevant dataset points somewhere else entirely. Lever, Frederick and Peplau surveyed 52,031 people in 2006 and found that 85% of women reported being satisfied with their partner’s size, while only 55% of men were satisfied with their own. The gap between those two numbers is the most consistently replicated pattern in this literature — though it, too, comes from self-reports rather than measurement. And it is not about girth or length. It is about the difference between how men see themselves and how their partners see them.
Can girth be changed?
Straight answer: there is no clinical evidence that manual training permanently increases girth. No controlled trial has demonstrated it. Men in training communities do report changes over months of consistent work — but those are self-reports from people motivated to see progress, collected without controls, and they should be weighted accordingly. If someone offers you a number of centimeters in a number of weeks, they are selling something that the published record does not support.

What to do with your number
Measure at mid-shaft, fully erect, with a tape, and write down where you measured. Convert if you have been using a ruler: multiply by 3.14. Then check the result against the nomograms rather than against a forum post.
What that sequence usually settles is the gap between the number you were carrying around in your head and the one on the tape. The distribution is narrower than most men picture it — that part the nomograms do show.
